Use CLTWater inspector's sketch for stationing on water mains and services and for sewer laterals, if applicable. Mark through and redraw when the as-built location is more than one inch (scaled – horizontally or vertically) from the proposed location. Along with the PE signature certifying the record drawing, you must have a note indicating who supplied the information – see attachment 'A'. Completed in ink and submitted on Mylar or Vellum. This map can be drawn on 11" x 14" or 11" x 17" paper and needs to be sealed, signed, and dated by an NCPLS. An overlay of any sewer mains that are in a recorded easement - This map should clearly depict the center of the as-built sewer and manholes on top of the recorded easement, as it is used to show that the main still falls within the recorded area. One copy of recorded easement agreements, if applicable. 
The other will be forwarded to our New Services Section when we accept the water and sewer mains in order to enter the addresses in our billing system for meters.
Two copies of recorded plats - One is for our Systems and Records Section to assist in processing the Record Drawings. A copy of the permit sheet is not required NOTE: We do not accept bond copies because there is no guarantee that the corrected Mylar submittals returned to us are the same as the bonds we reviewed earlier, resulting in double work for our staff. 
Always include the cover sheet - this helps our GIS Technicians know the general location when the drawing is sent to our internal Systems & Records Section. One reproducible set of pages – no bond copies accepted – only Mylar or Vellum will be accepted.
